Total thyroidectomy is one of the definitive treatments for Graves’ disease. This case describes the rare recurrence of thyroid eye disease (TED) and thyrotoxicosis due to thyroid remnant tissue. Overt hyperthyroidism occurs in about 0.1–0.4% of all pregnancies. Propylthiouracil (PTU) is recommended in first trimester due to risk of teratogenicity with Carbimazole (CBZ).
